Certified Medical Assistant - Chronic Care Management

We are seeking adetail-oriented and patient-focused Certified Medical Assistant to support ourChronic Care Management program. This role is responsible for enrollingeligible patients, coordinating ongoing care for an assigned panel, andensuring continuity of care for individuals with chronic conditions. Theposition blends patient outreach, care coordination, and in-clinic support toenhance patient outcomes and engagement. Key Responsibilities:<o:p></o:p><ul><li>Identify and enroll eligiblepatients into the CCM program, including educating patients on program benefitsand obtaining informed consent. </li><li>Manage an assigned panel ofCCM patients </li><li>Coordinate patient care incollaboration with providers, including appointment scheduling, specialistreferrals, medication refill requests, and follow-up on labs and diagnostictesting. </li><li>Support transitions of careby coordinating post-discharge follow-up, home care services, and communityresources to reduce avoidable hospital readmissions. </li><li>Serve as a patient advocateby addressing barriers to care, facilitating access to community and supportservices, and promoting disease prevention and self-management. </li><li>Accurately document allpatient interactions, care coordination activities, and administrativefunctions in the electronic medical record to ensure compliance with CCMprogram and billing requirements.</li></ul>Schedule & Compensation:<ul><li>Hourly Rate: $23–$25 perhour</li><li>Location: Amherst andNorthampton, MA (rotation between sites required)</li><li>Schedule: Monday–Friday, 40hours per week</li></ul>Qualifications:<o:p></o:p><ul><li>Certified Medical Assistantrequired</li><li>Minimum of 1 year ofexperience in a medical office or primary care setting</li></ul>
